Overview

Washington's medical cannabis program is governed by RCW 69.51A and administered by the Washington Department of Health (DOH). The program predates recreational legalization by over a decade, and it continues to provide significant advantages to qualifying patients — including a 37% tax exemption, tripled possession limits, home cultivation rights, and access to higher-potency products.

As of 2026, approximately 10,000 patients are registered in the system, and 302 of the state's approximately 460 retailers are medically endorsed to serve patients.

Critical Distinction: Authorization vs. Recognition Card

Washington has a two-tier medical system. A physician authorization alone provides minimal benefits (recreational amounts only, 4 plants, no tax exemption). You must complete the second step — registering for a recognition card at a medically endorsed store — to receive full medical benefits including 3x possession limits, 37% tax exemption, up to 6 or 15 plants, and access to higher-potency products.

Authorization vs. Recognition Card

This is the single most important distinction in Washington's medical cannabis program. Many patients stop after the first step and miss out on the majority of the program's benefits.

Authorization Only (Step 1)

A physician authorization is the first step, but by itself it provides limited benefits:

  • Possession limited to recreational amounts (1 oz flower, 7g concentrate)
  • Home cultivation limited to 4 plants
  • Possession of up to 6 ounces from home grow
  • No tax exemption — you pay the same 37% in taxes as recreational buyers
  • No access to higher-potency DOH-compliant products

Recognition Card (Step 1 + Step 2)

Completing both steps and registering in the database unlocks full medical benefits:

  • Possession of 3x recreational amounts (3 oz flower, 21g concentrate, 48 oz solid edibles, 216 oz liquid)
  • Home cultivation of 6 plants (or 15 plants with specific physician recommendation)
  • 37% tax exemption on all purchases at medically endorsed stores
  • Access to higher-potency DOH-compliant products not available to recreational customers
  • Entry into the state's medical cannabis database

13+ Qualifying Medical Conditions

To be eligible for a Washington medical cannabis authorization, a healthcare provider must certify that you have been diagnosed with one or more of the following conditions:

  1. Cancer
  2. HIV/AIDS
  3. Multiple sclerosis
  4. Epilepsy or seizure disorders
  5. Spasticity disorders
  6. Intractable pain
  7. Glaucoma
  1. Crohn's disease
  2. Hepatitis C
  3. Post-traumatic stress disorder (PTSD)
  4. Traumatic brain injury (TBI)
  5. Nausea, vomiting, anorexia, or cachexia
  6. Any condition the provider determines may benefit from cannabis treatment (broad catch-all)

The broad catch-all provision gives healthcare providers significant discretion. If a provider believes cannabis may benefit a patient's condition, they can issue an authorization even if the specific condition is not explicitly listed.

The Two-Visit Authorization Process

Washington requires a two-step process to obtain full medical benefits:

Step 1: Healthcare Provider Visit

Schedule an appointment with a Washington-licensed healthcare provider. The following provider types are authorized to issue medical cannabis authorizations:

  • Medical Doctors (MD)
  • Doctors of Osteopathy (DO)
  • Physician Assistants (PA)
  • Advanced Registered Nurse Practitioners (ARNP)
  • Naturopathic Doctors (ND)

The provider will conduct a physical examination and, if appropriate, issue a medical cannabis authorization on a tamper-resistant form. This is a legal requirement — the authorization must be on the specific tamper-resistant paper prescribed by the DOH.

Typical cost: $120–$200 for the provider evaluation.

Step 2: Medically Endorsed Store Visit (for Recognition Card)

To complete registration and receive your recognition card, visit one of the 302 medically endorsed retail stores in Washington. At the store:

  1. A certified cannabis consultant will review your authorization
  2. Your information is entered into the DOH medical cannabis database
  3. You receive your recognition card

Registration cost: Minimum $1 (set by the store; most charge the minimum).

New MCR System (June 30, 2025)

On June 30, 2025, the DOH launched a new Medical Cannabis Registry (MCR) system powered by VisualVault. This modernized system replaces the previous registry and introduces several improvements:

  • Electronic recognition cards — digital cards accessible through your online patient account
  • Online patient accounts — manage your registration, check card status, and update information
  • Help desk available 6 AM to midnight for technical support and patient questions

Tax Savings: Why the Recognition Card Matters

Washington's combined cannabis tax rate is approximately 37% (including the cannabis excise tax and applicable state/local sales taxes). Recognition card holders are exempt from this tax when purchasing at medically endorsed stores.

Recognition card holders enjoy three times the recreational possession limits:

Patients with authorization only (no recognition card) are limited to recreational possession amounts plus 4 home plants and up to 6 ounces from their home grow.

Home Cultivation for Medical Patients

Medical patients are the only people in Washington who can legally grow cannabis at home:

  • Authorization only: Up to 4 plants, up to 6 oz of usable cannabis
  • Recognition card: Up to 6 plants
  • Recognition card with specific recommendation: Up to 15 plants (requires physician documentation of need)

For recreational users, growing even one plant is a Class C felony.
